Introducing Our Programs

• Family Care
  • Family Care offers long-term care supports.
  • The member is at the center of the care team, and is supported by a care manager, RN care manager and other natural supports.
  • Eligible individuals must meet income requirements, live in a service county, and be 18 years or older with a qualifying disability.
  • Medicaid-eligible.
  • Must seek options counseling at local Aging and Disability Resource Center (ADRC) for enrollment.

• Partnership
  • Partnership offers long-term care supports and health and medical support.
  • The member is at the center of the care team and is supported by a care manager, an RN care manager, a nurse practitioner or physician assistant, and a team of administrative assistants.
  • A team of highly-qualified experts are available for consulting on care plans. This includes a medical director, pharmacists, and other behavioral and social work experts.
  • Eligible individuals are 18  and older with a qualifying disability who require nursing home level of care.
  • Medicaid and/or Medicare-eligible.
  • Must seek options counseling at local Aging and Disability Resource Center (ADRC) for enrollment.

• Medicare Dual Advantage
  • Medical Dual Advantage is a special needs Medicare advantage plan available to adults who qualify for both Medicaid and Medicare (adults with disabilities and those 65 or older).
  • The member is at the center of care and is supported by a care coordinator and nurse care manager.
  • Gives access to Medicare benefits such as primary care, hospitalization, and prescription drug coverage (Part D).
  • Members of My Choice Wisconsin Family Care and Medicaid SSI may also enroll in the Medicare Dual Advantage plan if they qualify for Medicare. (Partnership members cannot enroll in both plans.)
  • Individuals may enroll directly with My Choice Wisconsin; our Medicare Product Specialists will help assess eligibility and whether the program is right for you.
