On January 28, 2022 a memo went out to My Choice Wisconsin providers who have provided complex rehabilitation technology services for MCW members within the last 12 months. This memo explains the need for such providers to enroll under the new provider specialty, complex rehabilitation technology (CRT) supplier, by mandate of the WI Department of Health Services. Forward Health has also established clinical criteria for PA requests for CRT suppliers.
