Medicare and Partnership Member ID Card Error Impacting Member Eligibility Verification

IMPORTANT: Please share this information with your patient-facing staff who verify patient enrollment and eligibility.

Following our integration and new benefit year, we have been made aware of a print error on our recently-issued My Choice Wisconsin by Molina Healthcare Medicare Dual Advantage and Partnership Duals member ID cards. The group ID number is reflecting the Rx group ID number instead of the Health Plan group ID number.

We are working quickly to resolve this issue. In the meantime, if you are unable to identify a My Choice Medicare or Partnership member using their My Choice member ID or group ID, please remember you may verify member eligibility in ForwardHealth using any of the following options:

  • The member’s first and last name and date of birth,
  • Their Medicaid member ID, or
  • Their social security number and date of birth.
