Caring Starts with a Commitment to Privacy

My Choice Wisconsin takes your privacy very seriously. We adhere to the highest standard of care for your protected health information (PHI) and financial data, which means that we only share information with those who need it, when they need it (like your doctor).

We have a legal duty to you, our member, and to our funding partners, the Wisconsin Department of Health Services and the Centers for Medicare & Medicaid Services, to protect you. You inform us when we can use or disclose your information and we will follow those directions.

We encourage every member and/or their guardian to read more about your privacy rights. An informed member is an empowered My Choice Wisconsin member! Language translations of the Notice of Privacy Practices may be found in your program’s Member Resource Library.

If you have a privacy-related question or concern, contact My Choice Wisconsin’s Privacy Officer.
