Leah – Care Manager, Eau Claire

I am excited to be joining My Choice Wisconsin as a Care Manager! I obtained my master’s degree from UW-Stout in Clinical Mental Health Counseling. I have over ten years of experience working with individuals with diverse abilities, most recently by helping individuals meet their vocational goals. I am looking forward to now being able to assist members in meeting the goals they have set for themselves in all areas of their lives!

Shirley – Care Manager, Milwaukee

I am a Milwaukee native with 14 years of long-term care experience. I am the proud mom of three boys. In my spare time, my husband and I sing and play with my band. I am excited to be here because of the positive impacts that this program has on the community.

Caleah – Dedicated Functional Screener, Fort Atkinson

I’m a parent of two kiddos and a few fur babies! I really enjoy being active and getting out in nature! I don’t take any moment for granted and enjoy every day with gratitude and a positive outlook! I’m excited to join the team. 🙂
