2024 Partnership (Medicaid Only) Member ID Card

When you select My Choice Wisconsin for the Partnership program, you will receive your Member ID card.

If you are a My Choice Wisconsin Partnership member with both Medicare and Medicaid, your Member ID card will have the MedicareRx logo on the bottom of your card.
