Become a Contracted Provider with My Choice Wisconsin / Molina Healthcare of Wisconsin
As of May 1, 2025, My Choice Wisconsin by Molina Healthcare is reopening its Family Care and Family Care Partnership long-term care provider networks to new applicants.
In addition, providers interested in joining our network for Dual Advantage (D-SNP) may also apply.
To be eligible to apply, all new FC and FCP providers must:
- Be Home and Community-Based Services (HCBS) compliant, where applicable
- Be enrolled with Wisconsin Medicaid through the ForwardHealth Portal, where applicable
- Be eligible and appropriately licensed to provide services under applicable benefit programs, including Family Care, Family Care Partnership, or Dual Advantage (D-SNP), if applicable
(Note: As of May 1, 2025, My Choice Wisconsin / Molina Healthcare requires enrollment to be complete at the time of application for those provider types DHS designates as eligible to enroll. See detailed DHS enrollment guidance below.)
Please Note:
Although the Wisconsin Department of Health Services (DHS) requires all long-term care providers to be enrolled with Medicaid by January 1, 2026, My Choice Wisconsin / Molina Healthcare requires Medicaid enrollment to be complete at the time of application as of May 1, 2025.
This ensures timely processing and readiness for compliance with upcoming state requirements.
How to Apply
If you meet the above requirements and are interested in becoming a contracted provider for Family Care or Family Care Partnership, please follow these steps:
Step 1: Submit a Request to Apply
Complete the online form here:

You will receive a determination email within 30 days of submission.
Step 2: If Approved
You’ll receive an email from a contracting representative with:
- Required application documents
- Instructions to submit a full application within 30 days
Follow all steps carefully to avoid delays.
Step 3: Credentialing
The credentialing process may take up to 90 days.
You will receive a final outcome via email.

Step 4: If Denied
If your Request to Apply is not approved, you will receive an email with the reason and the timeframe in which you may reapply.
Certification Fees for 1-2 Bed AFHs
- New Certifications & Change of Address: $475
- Recertifications: $350
- Third Visit (if required): $75
Fees must be paid by check prior to scheduling the on-site certification appointment. Fees are non-refundable unless:
- MCW is unable to certify due to dual certifications (full refund)
- Provider does not pass credentialing (50% refund may apply)
- No refund will be issued if provider abandons the process
DHS Medicaid Enrollment Requirements
As a My Choice Wisconsin by Molina Healthcare provider or vendor, you are an important part of getting quality, timely care to Wisconsin adults who are older or have disabilities. We want to remind you about a new requirement for you to enroll with Wisconsin Medicaid through the ForwardHealth Portal by January 1, 2026.
Who Should Enroll Now:
- Organizations and agencies
- Sole proprietors
- Individuals who are not self-directed support workers
- Individuals who deliver supportive home care along with other services
Starting early allows you to be listed in the ForwardHealth Provider Directory and easily identify the services and programs relevant to you.
Who Will Enroll Later in 2025:
- Supportive home care organizations
- Providers with a unique Electronic Visit Verification (EVV) provider agency ID

- Tax identification number(s)
- National Provider Identifier (NPI), if applicable
