Medicare Dual Advantage (HMO SNP) is a Medicare Advantage plan for people who qualify for both Medicaid and Medicare. This plan helps coordinate your healthcare needs with no monthly premiums or deductibles. Your plan includes prescription medications, and you’ll have access to unique program benefits like dental and vision coverage and a monthly allowance to spend through our over-the-counter catalog.

This plan also has a care coordinator to help you keep track of your health. They partner with you and your healthcare providers to make sure you’re getting the right services. Members have access to a registered nurse via phone 24-hours a day. And we also have MCW subject-matter experts that can step in when needed. The care coordinator will work with you to:

• Access the doctors and treatments you need
• Coordinate appointments
• Use your benefits, including your supplemental benefits
• Set health and safety goals
• Connecting you to food, housing, and/or community resources
