We appreciate your interest in partnering with My Choice Wisconsin. Currently, we are experiencing a high volume of applications and our timelines have changed temporarily. My Choice Wisconsin will review applications within 30 days. Once the application has been reviewed and approved, the documents will be processed through our credentialing team. Credentialing can take up to 90 days depending on volume.
My Choice Wisconsin encourages small, minority-owned, women-owned, disadvantaged/disabled and veteran-owned business enterprises to become a part of our provider network.
My Choice Wisconsin seeks to ensure our members, including those with limited English proficiency, limited reading skills, hearing incapacity, or those with diverse cultural and ethnic backgrounds have access to providers that offer respectful, culturally competent care. We also seek to ensure our members, including those with specific mental, physical and/or developmental disabilities have access to providers and resources, including access to screening, diagnosis, referrals, and appropriate treatment of their disabilities.

How to Become a My Choice Wisconsin Provider

We are accepting applications to contract for the following provider services (Long term care – Acute & Primary Services):

  • Milwaukee County (GSR 8):
    • Residential Services: Licensed Adult Family Homes, CBRFs or RCACs that are non-ambulatory (must have entrance ramps) with an onsite Hoyer with staffing to accommodate non-ambulatory Hoyer services
    • Acute & Primary Services
    • All other services allowed under the benefit package
  • Dane County (GSR 12):
    • Residential Services: Licensed Adult Family Homes, CBRFs or RCACs that are non-ambulatory (must have entrance ramps) with an onsite Hoyer with staffing to accommodate non-ambulatory Hoyer services
    • Acute & Primary Services
    • All other services allowed under the benefit package
  • Rock County (GSR 14):
    • All Residential Services (please note that there is a fee for certification)
    • Acute & Primary Services
    • All other services allowed under the benefit package
  • Brown County (GSR 13):
    • All Residential Services (please note that there is a fee for certification)
    • Acute & Primary Services
    • All other services allowed under the benefit package
  • Sheboygan, Washington, Ozaukee, Waukesha, Walworth Counties (GSR 6):
    • All Residential Services (please note that there is a fee for certification)

  • Racine, Kenosha Counties (GSR 11):
    • All Residential Services (please note that there is a fee for certification)

  • Adams, Waushara, Marquette, Green Lake, Columbia, Dodge and Jefferson Counties (GSR 5):
    • All Residential Services (please note that there is a fee for certification)

  • Winnebago, Fond du Lac, Manitowoc Counties (GSR 9):
    • All Residential Services (please note that there is a fee for certification)

    • All other services allowed under the benefit package (Immediate needs: Transportation, Personal Care, Home Health, Supportive Visits, Supportive Home Care)
  • Juneau, Sauk, Richland, Crawford, Grant, Iowa, Lafayette, Green Counties (GSR 3):
    • All Residential Services (please note that there is a fee for certification)
    • Acute & Primary Services
    • All other services allowed under the benefit package (Immediate needs: Transportation, Personal Care, Home Health, Supportive Visits, Supportive Home Care)
  • Clark, Jackson, Trempealeau, Pepin, Buffalo, Jackson, LaCrosse, Monroe, Vernon Counties (GSR 2):
    • All Residential Services (please note that there is a fee for certification)
    • Acute & Primary Services
    • All other services allowed under the benefit package (Immediate needs: Transportation, Personal Care, Home Health, Supportive Visits, Supportive Home Care)
  • Douglas, Bayfield, Ashland, Iron, Price, Taylor, Burnett, Washburn, Sawyer, Polk, Barron, Rusk, St Croix, Dunn, Chippewa, Taylor, Pierce, Eau Claire Counties (GSR 1)
    • All Residential Services (please note that there is a fee for certification)
    • Acute & Primary Services
    • All other services allowed under the benefit package (Immediate needs: Transportation, Personal Care, Home Health, Supportive Visits, Supportive Home Care)

Due to the importance of following processes and vetting of potential providers, please be advised that the credentialing process can take up to 90 days. It is imperative that providers work directly with our credentialing department and return mandatory documents within 30 days of application receipt. If you are requesting a status on your credentialing application, please reach out to our credentialing department at credentialing@mychoicefamilycare.com. Please be advised that once you have received an email from the credentialing department stating that you have passed, the assigned contractor will reach out to you within 7 business days.

Fees for a 1-2 Bed AFH:

  • New Certifications & Change of Address $475.00.
  • Recertifications $350.00.
  • 3rd Visit of certifications $75.00.
  • The Credentialing Department will remind you of this fee and fee must be paid by check and prior to a certifier being deployed to your location.
  • Fees may not be refundable under these circumstances but not limited to:
    • MCW is unable to certify due to dual certifications.
    • 50% of fee may be refunded if provider does not pass credentialing.

Fees will not be refunded if provider abandons the certification process for any reason.

To change the Tax Id or name on your contract, please fill out the change request form and email to the credentialing department at shfamccontracts@mychoicewi.org.

In the summer of 2019, My Choice acquired Trilogy Health Insurance and on June 1, 2021 rebranded it under My Choice Wisconsin Health Plan, Inc.

What does this mean for you? Beside the name change to My Choice Wisconsin Health Plan, Inc. – nothing. There are no changes to contracts, to the process for authorization requests, or your contacts.
