How to Become a My Choice Wisconsin Provider
Thank you for your interest in joining the My Choice Wisconsin by Molina Healthcare long term care provider network. At this time, we are not accepting any new applications for long term care services, including both new service providers and existing provider requests to add services or facility locations to current contracts. Please check back on this page for updates on when we resume accepting applications.
- Milwaukee County (GSR 8):
- Residential Services: Licensed Adult Family Homes, CBRFs or RCACs that are non-ambulatory (must have entrance ramps) with an onsite Hoyer with staffing to accommodate non-ambulatory Hoyer services
- Acute & Primary Services (Medicare and Partnership)
- All other services allowed under the benefit package
- Dane County (GSR 12):

Due to the importance of following processes and vetting of potential providers, please be advised that the credentialing process can take up to 90 days. It is imperative that providers work directly with our credentialing department and return mandatory documents within 30 days of application receipt. Please be advised that once you have received an email from the credentialing department stating that you have passed, the assigned contractor will reach out to you within 7 business days.

Fees for a 1-2 Bed AFH:
- New Certifications & Change of Address $475.00.
- Recertifications $350.00.
- 3rd Visit of certifications $75.00.
- The Credentialing Department will remind you of the fee for Certification. Thefee must be paid by check prior to scheduling an on-site Certification appointment.
- Fees are not refundable. A refund or partial refund could occur under the below circumstances:
- MCW is unable to certify due to dual certifications.
- 50% of fee may be refunded if provider does not pass credentialing.
Fees will not be refunded if provider abandons the certification process for any reason.
