- Call 1-800-963-0035 if you would like help choosing a provider or need a printed copy of the provider directory.
- Visit our pharmacy locator to find one nearby.
- Call 1-800-963-0035 for help finding a pharmacy near you, to coordinate mail order service, or if you need a printed copy of the pharmacy directory.
- This benefit provides you with a monthly allowance ($155/month for the 2024 plan year) to use when purchasing over-the-counter items from our catalog or at network retail locations. This way, you don’t have to spend your own money on things like vitamins, cold and allergy medicines, toothpaste, incontinence pads, and first aid. Plus, they will be delivered right to your door!
- Any unused allowance will roll over to the next month. Your OTC allowance must be used by the end of the year (12/31/2024 for the 2024 plan year).
- Your OTC catalog will arrive to you in the mail three weeks after your enrollment effective date.
- You may order by phone, mail, or online. Instructions on how to place your order over the phone or by mail can be found in the catalog. To order online, use the online portal.
- Your OTC card will arrive to you in the mail in a separate envelope three weeks after your enrollment effective date. This card may be used to purchase covered items at network retail locations. To find a participating retailer near you, call 855-816-9469. (TTY 711)
- Your supplemental vision benefit is in addition to vision coverage you already have with Medicaid. This supplemental benefit provides $150/year towards eyeglass lenses, eyeglass frames, eyewear upgrades, or contact lenses. Keep this in mind, as you will be responsible for any balance not covered by your benefits.
- If you’re ready for new eyeglasses or contacts, find an in-network provider by searching “Optometry” on the online provider directory or by calling 833-479-0201.
- Show your in-network vision provider your My Choice Wisconsin Medicare Dual Advantage member ID card. They can then call our Vision benefit center toll-free at 833-479-0201 to get an authorization.
- This benefit covers up to $2,500 in dental services such as:
  
  Two diagnostic visits per year
  
  Oral exams, cleanings, and one set of x-rays
  
  Restorative services like crowns, fillings, up to one restoration per tooth, per year
  
  Dentures
  
  Non-surgical extractions
  
  Laboratory-made porcelain crowns
- Your supplemental dental benefit is in addition to dental coverage you already have with Medicaid.
- Show your provider your My Choice Wisconsin Medicare Dual Advantage member ID card. They can call DentaQuest 1-833-479-0201 to get an authorization.

- Contact your Care Coordinator by calling our toll-free Customer Service line at 1-800-963-0035.
- They will ask you a few questions to place the order.
- PERS units usually arrive in 2 weeks.
- This benefit provides you with two meals per day for up to 28 days (maximum of 56 meals) following an inpatient hospital or skilled nursing facility (SNF) stay. An unlimited number of events/occurrences are covered in the calendar year.
- You, your provider, or someone else on your behalf can request this benefit by contacting your My Choice Wisconsin Care Coordinator. If you need help connecting with your Care Coordinator, please call our Customer Service Center tollfree at 1-800-963-0035.
- Your supplemental Home and Bathroom Safety Devices and Modifications benefit provides up to $300 per year toward the purchase and installation of the following items to help keep you safe in your bathroom: grab bars, shower chairs, bathtub benches, raised toilet seats, toilet chairs, and handheld shower heads.
- This benefit is intended to improve safety of the bathroom in your private home. It is not for improvements made to bathrooms in a group living situation or facility.
- If you need any of these items, please contact your Care Coordinator by calling 1-800-963-0335 and requesting to speak with your Medicare Dual Advantage Care Coordinator. Your Care Coordinator will discuss which items you need, as well as how to obtain them and get reimbursement.
- Any installation must be completed by a licensed contractor.
