As we shared in January, we were made aware of a print error on member ID cards for My Choice Wisconsin by Molina Healthcare Medicare Dual Advantage and Partnership Duals members. We have worked quickly to resolve the issue with our vendors and reissue ID cards.
Next week, all impacted members will receive new ID cards in the mail, with a note to discard their old cards.
You may begin to see members presenting with new ID cards where the Health Plan group ID number has been removed.
If you are still unable to identify a My Choice Medicare or Partnership member using their My Choice member ID or group ID, please remember you may verify member eligibility in ForwardHealth using any of the following options:
- The member’s first and last name and date of birth,
- Their Medicaid member ID, or
- Their social security number and date of birth.
