Gwen – Care Manager, Madison

I recently moved to Madison, Wisconsin from Illinois. In Illinois, I worked in a group home providing direct care, and then moved to a community day service program where I worked as a case manager. I’m excited to learn more about care management through my new job at my choice Wisconsin.

Chris – Care Manager, Madison

I grew up near Madison and have always enjoyed working with people. I love live music and spending time with my wife and 16 year old. I own six pet birds and serve on the board of a nonprofit parrot rescue in Edgerton.

Dayna – Care Manager, Dodgeville

I hold a bachelor’s degree in Psychology with a minor in Sociology. I have fifteen years of experience working in the human service field, as I’ve worked in areas such as child-welfare, housing, as well as with individuals who’ve had intellectual and/or physical disabilities. I have a passion for supporting others through advocacy and being of service throughout their growth. I’m an avid reader who also enjoys running and spending time with my family and our three dogs. I look forward to being a part of the My Choice Wisconsin team!

Jeanmari – RN Care Manager, Madison

I’m an enthusiastic person about living life to its fullest ! I believe that I can contribute much to helping those members that I serve. I love to teach and enhance the well being of others.

Felicia – Dedicated Functional Screener, Wauwatosa

I have extensive background working with people who have disabilities. I look forward to learning from clients as a Functional Screener to allow me to better understand challenges with their daily activities. Coupled with my Bachelors Degree in Social Work, I can better access their needs.

Talayla – Care Manager, Milwaukee

I am very adventurous easy to approach with a great sense of humor. I love sports and love do-it-yourself crafts projects. I am a huge foodie and love spending time with my daughter.

Darla – Care Manager, Milwaukee

I am someone who loves working in the human service field and being able to help individuals in need of services. I am excited to be given the opportunity to join My Choice Wisconsin, and I look forward to providing quality care to our members in the Wisconsin area.

My love for humanity made me reconsider my career. I quickly switch from journalism and marketing to nursing where I will have an impact on lives, especially the vulnerable people who can’t advocate for themselves. Touching lives and leaving a positive mark makes me feel accomplished. I am happy to be joining this team where Together we will make this possible.
